We are looking for motivated, flexible and committed individual to join the team in our young people service in Westminster. The service are for Care Leavers aged 16 -21 yrs old.

The service will form part of the existing Young Persons Pathway.

About The Role

Our aim is to improve the quality of life of our customers, building confidence and skills to allow them to move on independently, while playing an active role in their communities. We view all our customers as individuals with specific needs, treating them with respect and dignity, and focusing on achieving real results.

The length of stay is 9 months and is classed as a low/medium needs services. 

Staff are required to work on a rota basis, including evenings and weekends.

About You

The candidates must have experience of working with young people in a support capacity. This includes experience and knowledge of assessment and support of young people.

The service will be for vulnerable young people with a variety of needs. The successful candidates will carry out assessments in collaboration with Support Officers. This includes delivering solution focused, flexible packages of support to assist each young person to develop independent living skills necessary for successful move on.

Critical to the success of the service will be proactive, professional, and effective partnership working with a variety of external partners. Such partners include our referral agents, employment and training partners, and specialized service partners.

Training will be required to be attended alongside daily scheme responsibilities.

As a Support Assistant you will be expected to:

  • Work on a rota basis including evenings and weekends
  • Ensure the service is running at a high standard and meeting all the service specification outlined by commissioners.
  • Raise customer awareness of local services and facilities and support engagement with these services. This includes One Housing’s Employment & Training Team and Volunteer & Partnership Team.
  • To carry out support planning, risk assessments and reviews for lower need customers.
  • To provide a broad range of lower-end support tasks to a caseload of customers, such as monitoring and tackling social isolation and ensuring customers are well supported throughout their engagement with the service.
  • Undertake assessments, with support of a Support Officer.
  • Dealing with incidents and safeguarding concerns in accordance with organizational policies and procedures.
  • Prepare clear and concise documents and reports as required. This includes all documentation for quality standard evidence and written handovers.
  • Representing the service at external local authority/provider panels and conferences.
  • Facilitate and encourage effective customer participation, consultation and feedback within the service.
  • Take a lead on promotion and encouragement of involvement in Social Inclusion and Duke of Edinburgh’s Award.
  • Develop a wide range of activities, including group sessions, social events, workshops, and group activities, designed to provide young people with additional life skills.
